Athletic Clearance Process

As mandated by the FHSAA and Bishop Kenny High School, each student is required to submit a valid FHSAA physical, EKG, concussion baseline test, and NHFS videos PRIOR to participating in any BKHS athletic activity.

Below are the individual steps to complete so your student is ready to participate.

Athletic Forms – 2026–2027 School Year

Athletic and participation forms for students participating in sports will be available beginning May 22, 2026, via Final Forms.
 

1.   Final Forms Registration

Please click HERE to register or log in to Final Forms.
If you have not already registered, choose Login and then select Resend Confirmation. The confirmation will be sent to your email so you can set up your account.
 

2.   Physical Examination (EL2 Form)

All student-athletes are required to upload a copy of their current physical exam (EL2) to Final Forms.

• The exam must be dated on or after May 22, 2026, for the 2026–2027 school year.
• The EL2 form is available here: EL2
• Please print the form and have your doctor complete all required information, including signature, stamp, and date.
• Upload the completed EL2 form to Final Forms.
• Check back for approval or denial message of the EL2

Baseball and Softball Players:
Please wait until after June 1, 2026, to complete your physical due to the length of the season.
 

3.   ECG Screening  (Incoming 9th Graders or Students Trying out for a sport for the 1^st time only)

Per the new FHSAA policy for 2026, all incoming 9th graders are required to complete an ECG screening before participating in practices or tryouts. Any students in grades 10-12 who are trying out for a BKHS sport for the first time will also need an ECG. Returning athletes who have already played for BKHS are exempt.

• Please print the form and have your doctor complete all required information, including signature, stamp, and date.
• Upload the completed EL1 form to the ECG screening section in Final Forms.
• The actual ECG test results do not need to be uploaded — only the completed EL1 form.
   

4.   Concussion Baseline Testing

As required by Bishop Kenny High School, each student-athlete must complete a valid Concussion Baseline Test every two years.

• Athletes entering their junior year must complete the Concussion Baseline Test again this year.

New and Returning Athletes:

Prior to participating in conditioning or tryouts, all student-athletes must:

• Complete and submit all Final Forms electronically
  • This includes the NFHS videos embedded in the Final Forms checklist.
• Upload their Physical Exam Form (EL2) and receive approval.
• Upload their ECG Screening Form (EL1), if applicable.
• Complete a valid Concussion Baseline Test
